Because we take your privacy rights seriously, we will not ask for your records before your first visit here, as you are not yet a patient of our office. You must request them and bring them to your first appointment and/or send them to us.

If you feel your HIPAA rights have been violated, you have the right to file a complaint with our facility's privacy contact, who is listed with our privacy notices or you may contact the Department of Health & Human Services (Office of Civil Rights) or both.
